Job Overview
We are seeking a dedicated and energetic In-Home Care Provider to join our compassionate team. In this vital role, you will deliver personalized care and support to clients in their own homes, helping them maintain independence and quality of life. Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health through observation of vital signs and overall condition
- Administer medications accurately following medical instructions and documentation protocols
- Support clients with meal preparation, light housekeeping, laundry, and cleaning tasks to promote a safe and comfortable environment
- Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document care activities and medical records precisely
- Employ proper patient transfer methods using equipment such as Hoyer lifts or transfer belts to ensure safety during mobility assistance
- Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients and their families
Qualifications
- Proven experience working with seniors or individuals with disabilities in home healthcare or assisted living settings
- Knowledge of medical terminology, medication administration, catheter care, and basic life support (CPR/First Aid)
- Familiarity with home health care regulations and documentation procedures using EMR/EHR systems
- Strong skills in patient observation, patient personal care assistance, and dementia or Alzheimer’s care
- Ability to perform heavy lifting safely when necessary and work independently in clients’ homes
- Excellent communication skills for client interaction and family updates
- Valid certifications in CPR and First Aid; additional certifications such as hospice or geriatrics are a plus
- Experience working with geriatric patients or those with developmental disabilities is highly valued
