Job Summary
We are seeking a dedicated and compassionate Home Health Aide/Certified Nursing Assistant to deliver exceptional in-home care to our clients. The ideal candidate will possess a strong background in patient care, with experience working with diverse populations including seniors, individuals with disabilities, and those requiring specialized dementia or hospice care. This role involves assisting with daily activities, monitoring health status, and ensuring a safe and supportive environment for our clients.
Responsibilities
- Provide personal care including bathing, grooming, dressing, and continence management while respecting client dignity.
- Assist with activities of daily living such as meal preparation, light housekeeping, laundry, and transportation to appointments or social outings.
- Monitor vital signs and observe patient conditions to report any changes or concerns promptly.
- Support clients with mobility assistance using equipment like Hoyer lifts or transfer belts to promote safety.
- Administer medications and perform basic medical tasks such as catheter care under supervision or as directed by healthcare professionals.
- Maintain accurate medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA regulations.
- Engage clients in companionship activities and life enrichment exercises to promote mental well-being.
Skills
- Proven experience in home healthcare settings, assisted living facilities, or nursing homes.
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring.
- Experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease, or end-of-life care needs.
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation procedures.
- Ability to perform heavy lifting safely and operate mobility aids such as Hoyer lifts or wheelchairs.
- Excellent communication skills for effective interaction with clients, families, and healthcare teams.
- Knowledge of home & community care regulations and best practices for geriatric and hospice patients.
