Unit: Hospice Case Manager
Experience Required: 2 years of Hospice experience needed; will accept first time traveler if they have Hospice and EPIC Remote Client Experience.
Certs Required: BLS, Valid DL and Insurance
Skills Required: End of life care/planning, wound assessments, care coordination, pain/symptom management, triage
Floating: Anywhere within the counties
Patient Types: Adults only – End of Life patients
Ratios: 1:1
Scheduling:
Weekend rotation: One weekend a month
Call? One call shift a month
Holiday Expectations: No holidays
Approval of time off: Can we approve time off on your behalf, or would you like all requests sent to you? Manager to approve all RTO
Shift times: M-F 0800-1630
