Description:
As a Hospice Registered Nurse (RN), you will provide compassionate, end-of-life care to patients in their homes or hospice facilities, focusing on comfort, dignity, and support. You'll work closely with patients, families, and interdisciplinary teams to manage symptoms, ease pain, and ensure emotional and spiritual support during a profoundly important time. This role requires not only strong clinical skills but also empathy, patience, and the ability to provide guidance through difficult conversations and decisions.
Qualifications
Current RN license, BLS (other certifications as required by facility), Two years of recent Hospice RN experience, Strong communication and adaptability skills
Responsibilities
Assess patients’ conditions and develop individualized care plans centered on comfort and symptom management., Administer medications and treatments to manage pain and other distressing symptoms., Provide emotional support and education to patients and families, helping them understand the hospice journey., Collaborate with physicians, social workers, chaplains, and other members of the care team., Monitor patient progress and adjust care plans as needed., Maintain accurate clinical documentation in accordance with hospice regulations and standards., Offer compassionate presence and reassurance during the final stages of life.
