Position Summary
Total Rehab at Moorestown is seeking a dedicated and detail-oriented Admissions Nurse for the 3:00 PM – 11:00 PM shift. This role is essential in ensuring a smooth, welcoming, and clinically sound admission process for new residents. The ideal candidate is compassionate, organized, and experienced in skilled nursing or post-acute care settings.
Admissions Nurse Key Responsibilities
- Coordinate and complete all aspects of resident admissions during the shift
- Perform initial nursing assessments upon admission
- Review and verify physician orders, medications, and treatment plans
- Collaborate with interdisciplinary team members to ensure continuity of care
- Communicate effectively with residents, families, and referral sources
- Ensure all documentation is completed accurately and in compliance with regulations
- Facilitate a positive admission experience for residents and families
- Address immediate clinical needs and escalate concerns as necessary
Admissions Nurse Qualifications
- Active RN or LPN license in the state of New Jersey
- Previous experience in a skilled nursing facility or admissions role preferred
- Strong assessment and clinical skills
- Excellent communication and interpersonal abilities
- Ability to work independently in a fast-paced environment
- Knowledge of EMR systems and admission protocols
Admissions Nurse Schedule
- Evening Shift: 3:00 PM – 11:00 PM
- Weekend rotation may be required
