What You'll Do
- Partner with a Board-Certified PM&R physician to manage patients in sub-acute rehab and skilled nursing facilities.
- Provide on-site pain management and rehabilitation-focused care to improve patient functionality and comfort.
- Collaborate with facility therapy departments to align medical and rehab plans of care.
- Coordinate care and communicate regularly with primary care providers, therapists, social workers, nurses, and families to ensure cohesive treatment plans.
- Complete timely, accurate charting and documentation, with flexibility to perform documentation tasks from home.
- Within the first 6–12 months, you are a trusted clinical partner to facility staff, reliably managing a panel of rehab patients with minimal supervision.
- Patient functionality and satisfaction scores improve through consistent, thoughtful pain management and rehab-focused interventions.
- Readmission rates related to pain and functional decline are reduced through proactive, on-site care and early intervention.
- Communication with families and interdisciplinary teams is timely and clear, resulting in coordinated care plans and smooth transitions.
- Licensed and certified Nurse Practitioner or Physician Assistant with active Tennessee licensure.
- Strong interest in Physical Medicine, Rehabilitation, or geriatric care within skilled nursing or sub-acute rehab settings.
- Detail-oriented, outcome-focused clinician who values teamwork, stability, and people-centered care.
- Reliable transportation and the ability to work in-person at skilled nursing facilities while managing documentation efficiently.
