In this role, you will partner with our PM&R physician in the sub-acute rehab setting. Location: In-person at skilled nursing facilities
Schedule: Flexible options, Part-time position available with potential to become full-time

Responsibilities: Partner with the facility's therapy department to provide the best possible rehab care. Communication and collaboration with the patient's primary care team to ensure the best outcomes. Timely charting/documentation is required but can be done from home.
