What You'll Do
- Partner with a PM&R physician to evaluate, treat, and manage patients in sub-acute rehab settings
- Collaborate daily with therapy teams (PT/OT/ST) to align medical care with functional recovery goals
- Coordinate care with primary care providers, nursing staff, social workers, and families
- Deliver on-site pain management and rehabilitation-focused treatment plans
- Complete timely and accurate documentation (with flexibility to finish charting remotely)
- Contribute to improving patient satisfaction, functional independence, and continuity of care
- Patients under your care demonstrate measurable gains in mobility, function, and independence
- Reduction in avoidable hospital readmissions through proactive care and coordination
- Strong collaboration with interdisciplinary teams leading to consistent, aligned care plans
- High-quality, timely documentation that supports continuity and compliance
- Positive feedback from patients, families, and facility partners
- Licensed and certified Nurse Practitioner or Physician Assistant (required)
- Active Tennessee license
- Ability to work independently while collaborating across interdisciplinary teams
- Strong clinical judgment in post-acute or rehabilitation settings (preferred but not required)
