Registered Nurse (RN) — Director of Case Management (Hybrid)
Salary Range $95,000 to $ 110,000

Schedule: Monday–Friday, 8:30 a.m.–5:00 p.m. (flexible) — Hybrid role: part onsite, part remote
Reports To: Deputy Director of Case Management Programs

This hybrid leadership opportunity allows you to apply your RN background beyond bedside care while still making a meaningful, measurable impact on participant health and safety. You will guide the Medical Day Care Case Management Program, ensuring high‑quality, person‑centered service delivery for Maryland’s Medical Day Care Services Waiver participants.
What You’ll Do
- Lead & mentor teams: Provide leadership and supervision to Assistant Directors, QA Managers, Supervisors, and Case Managers; serve as a key voice in executive meetings and with external partners.
- Clinical oversight—without bedside ratios: Ensure medical and behavioral health needs are met, oversee Plans of Care/Service, and review clinical documentation for accuracy and appropriateness.
- Coordinate interdisciplinary care: Collaborate with physicians, therapists, caregivers, Day Care centers, and community providers to support stable and safe transitions of care.
- Ensure quality & compliance: Maintain adherence to COMAR and Medicaid requirements; lead audits, QA/QI initiatives, and corrective actions; review billing documentation for compliance.
- Risk & safety leadership: Manage reportable events, incident reporting systems, and secure HIPAA‑compliant transitions of care.
- Elevate team capabilities: Provide clinical training, documentation coaching, emergency response guidance, and ongoing competency development.
- Communicate effectively: Serve as a clear and professional point of communication for families, participants, internal staff, and agency partners.
- Respond to urgent needs: Occasionally respond to medical, behavioral, or operational emergencies requiring leadership support.
Qualifications
- RN required: Active Maryland Registered Nurse license.
- Bachelor’s degree required; Master’s preferred.
- 3–5 years of nursing experience in adult health, community‑based care, chronic condition management, or day program environments.
- Supervisory experience within clinical or human‑services settings.
- Familiarity with Maryland Medical Day Care, Medicaid policies, and community‑based service models; LTSSMaryland preferred.
- Strong triage, assessment, care coordination, and communication skills.
