Case Manager-Support Planner & Coordinator of Community Services

This is a full-time, on-site position located in Maryland, United States. The Case Manager-Support Planner & Coordinator of Community Services will coordinate and oversee individualized plans to ensure that people with intellectual and developmental disabilities receive appropriate services to meet their unique needs. Responsibilities include conducting assessments, developing support plans, collaborating with families and service providers, monitoring progress, and maintaining accurate records. The role requires adherence to state regulations and a focus on promoting community inclusion and overall well-being.

• Experience in case management, support planning, and service coordination
• Strong interpersonal skills, including the ability to communicate and collaborate effectively with individuals, families, and service providers
• Proficiency in maintaining detailed records, documentation, and compliance with regulatory requirements
• Knowledge of resources and services available for individuals with intellectual and developmental disabilities
• Problem-solving, decision-making, and organizational skills
• Proficiency in the use of relevant software and technology
• Bachelor's degree in Social Work, Human Services, Psychology, or a related field
• Reliable transportation and a valid driver’s license
• Familiarity with Maryland's healthcare and social services systems is a plus
