Job Description
We are a growing home health agency in El Centro, CA, seeking a motivated and bilingual Part-Time Patient Outreach Specialist to support our office operations. In this role, you will be the frontline communicator, handling phone inquiries, conducting outreach (sometimes off site) for potential new patients, and managing interactions with existing patients to ensure smooth service delivery. Key Responsibilities:
- Answer incoming calls from patients, families, and referral sources, providing information about our home health services.
- Perform outreach via phone and email to connect with potential new patients, including following up on leads from doctors, hospitals, and community partners.
- Assist existing patients with scheduling, inquiries, and coordination of care needs.
- Liaise with other doctor offices, medical facilities, and healthcare providers to facilitate referrals and patient transitions.
- Maintain accurate records of communications and update patient information in our systems.
- Support general office tasks as needed to keep operations running efficiently.
This is a 1099 independent contractor position, offering flexibility in scheduling to fit your lifestyle. We value work-life balance and can accommodate varying availability.
Requirements
- Fluency in Spanish and English (both verbal and written) – required for effective communication with our diverse patient base.
- Strong phone etiquette and customer service skills.
- Comfortable with outreach and relationship-building in a healthcare setting.
- Basic computer proficiency (e.g., email, Microsoft Office or Google Workspace).
- Reliable transportation to our El Centro office.
Preferred Qualifications
- Experience in home health care or a similar medical environment (a plus, but not required – we provide training).
- Background in coordinating with doctor offices, clinics, or other medical providers.
- Outgoing personality with excellent interpersonal skills.
