As the Speech Language Pathologist, you will be responsible for evaluating and treating residents with communication and swallowing disorders to help them improve their quality of life and functional abilities.Responsibilities:- Evaluate residents' communication and swallowing abilities using standardized assessments- Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders- Provide skilled speech therapy interventions, including articulation and language therapy, cognitive-communication therapy, and swallowing therapy- Monitor and document resident progress and adjust treatment plans as necessary- Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and physical therapists, to coordinate care and ensure optimal outcomes for residents- Educate residents and family members on speech therapy goals, treatment plans, and home exercise programs- Maintain accurate and timely documentation of speech therapy services provided- Participate in team meetings, in-service education, and quality improvement initiatives- Maintain current knowledge of speech therapy practice and relevant regulations Qualifications:Current license to practice as a Speech Language Pathologist in the state of employmentMinimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facilityKnowledge of current speech therapy practice standards and regulationsStrong communication and interpersonal skillsAbility to work effectively as part of a teamAbility to manage multiple priorities and meet deadlinesProficient in using electronic medical records and other healthcare softwareWe are committed to providing the highest quality care to our residents, and we believe that our Speech Language Pathologist plays a critical role in achieving this goal.
