What are the responsibilities and job description for the Community Support Specialist / Systematic Skill Builder (Adults with Autism) position at Torch Elite Coaching?

This position of combined roles are up to 32 hours with the option for increasing the hours in the future as cases open up. This position is a hybrid with our other role of Systematic Skill Building.

The Systematic Skill Building role must possess a bachelor's degree (See Below) and has about 6 hours.

You may also still apply for the community support position which only requires a high school diploma, but the hours are about 24-26 currently.

This position is serving the areas around Harrisburg, Hershey, Enola, Palmyra, and Linglestown.

Our consumers are adults ages 21 or older with autism spectrum disorder (ASD) and their families through the Pennsylvania State Program called the Adult Autism Waiver (AAW). The program is designed to help adults with ASD participate in their communities in their own way based upon their identified needs.

We aim to provide the best service to our consumers so that they may live to express their personal interest and healthy lifestyles.

o This business is based in Columbia Pennsylvania, but you would be driving from your home to meet with the clients in their homes and or community locations. The areas we serve are Adams, Berks, Cumberland, Dauphin, Franklin, Lancaster, Lebanon, Northumberland, Perry, and York.

We will pair you with clients that are within an approximate 45-minute commute or less.

Responsibilities:

o Community Support Specialist (CS):

· Facilitate the directive of the “Skill Building Plan” (SBP) from the Skill Builder Specialist (SB).

· Assist participants in acquiring, retaining, and improving adaptive skills through coordinated goals attained from their ISP (Individual Support Plan) laid out by the SB.

o Responsibilities:

· Facilitate social interaction and participation in community activities.

· Improve capacity to perform daily living activities and instrumental activities (communication, cooking, housework, shopping, etc.) (in accordance with the SBP)

· Support participants in managing their medical care, including scheduling and attending appointments, filling prescriptions, and monitoring health.

· Collect data per session based on the criteria of the SBP and use a data collection sheet to communicate with the Skill Builder on progress.

· Using “Jot Form,” report the activities and goals attainment of the participant from the session.

· Report the participants visit start and ending time using “EVV” (Electronic Visit Verification) with the Sandata EVV Program.

· Communication with teams regarding the participants’ activities as needed to:

§ Internally:

· Chief Executive Officer (CEO)

· Chief Operating Officer (COO)

· Chief Services Officer (CSO)

· Behavior Support Specialist (BS)

· Systematic Skill Builder (SSB)

§ Externally:

· Parents or Legal Guardians (Often)

· Support Coordinator (Rarely)

· Bureau of Autism Representatives (Rarely)

o Requirements:

· High School Diploma or GED.

· Willingness to travel to clients' homes using your own reliable transportation with valid insurance.

· Proficient with Microsoft Office products.

· Mandated Person Check: FBI Clearance, Criminal Background.

Systematic Skill Building (SSB):

· Facilitate the directive of the “Skill Building Plan” (SBP) to help participant acquire skills for independence and community integration.

· Involves aligning the participants supports for consistent SBP implementation

o Responsibilities:

· Conduct evaluations related to existing or developing goals in the ISP of participant’s abilities and learning style.

· Develop “Skill Building Plan” (SBP) based on participant's goals within 60 days of start date.

· SBP includes benchmarks for assessing progress of the participant.

· Include participant’s history with skill acquisition and baseline skills.

· Provide direct and consultative support based on the SBP.

· Using “Jot Form” to report the activities and goals attainment of the participant from the session.

· Analyze data from the direct sessions and create and maintain a goal attainment chart for every goal in the SBP.

· Use the goal attainment chart data collection sheet to complete a quarterly progress note to support coordinators about how the SBP is being implemented.

· Complete the Quarterly Progress note from the Community Support Specialist’s (CS) data.

· Communication with teams regarding the participants’ activities as needed to:

· Internally:

o Chief Executive Officer (CEO)

o Chief Operating Officer (COO)

o Chief Services Officer (CSO)

o Behavior Support Specialist (BS)

o Community Support Specialist (CS)

· Externally:

o Parents or Legal Guardians (Often)

o Support Coordinator (Rarely)

o Bureau of Autism Representatives (Rarely)

o Requirements:

· Have a bachelor’s degree or higher in Social Work, Psychology, Education.

· Willingness to travel to clients' homes using your own reliable transportation with valid insurance.

· Proficient with Microsoft Office products.

· Mandated Check: FBI Clearance, Criminal Background.

Schedule:

o Monday to Friday (No nights)

o Flexible schedule

Benefits: (After 90 Days - if applicable)

o Mileage reimbursement for travel days

  • Travel Reimbursement: $20/ Day

Paid Training:

  • Orientation & Shadow Training $20/ Hour
  • Annual 24-hour Training after your first year
  • Monthly Paid Supervision: $20/ Hour

o Paid time off:

o 24-31 hours per week:

  • Holiday Pay: (6) 8-hour days (New Year’s Day, Memorial Day, July 4, Labor Day, Thanksgiving, Christmas Day)

OR

  • Floating PTO: – 48 hours but no holiday pay

*Roll over up to 40 hours for the next year

o 32 hours per week:

  • Holiday Pay: (6) 8-hour days (New Year’s Day, Memorial Day, July 4, Labor Day, Thanksgiving, Christmas Day)
  • Floating PTO: 40 hours

*Roll over up to 40 hours for the next year

o Positive performance yearly review increases pay and or benefits.
