Job Summary
We are seeking a dedicated and compassionate Home Health Licensed Vocational Nurse (LVN) to provide exceptional patient care in a home health setting. As a Home Health LVN, you will deliver personalized nursing care, educate patients and families, and collaborate with interdisciplinary teams to ensure comprehensive treatment plans are effectively implemented. Duties

  • Perform thorough patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
  • Administer medications via injections, IV infusions, and other routes with precision and adherence to safety protocols.
  • Provide wound care, catheterization, tube feeding, and other specialized nursing procedures following aseptic techniques.
  • Monitor patients' conditions closely, recognizing signs of deterioration or complications such as acute pain or infectious disease symptoms.
  • Educate patients and caregivers on medication management, disease processes—including Alzheimer's disease and dementia care—and self-care techniques.
  • Collaborate with physicians, case managers, and other healthcare professionals using electronic health record (EHR) systems like Epic or Cerner to coordinate comprehensive care.
  • Document all patient interactions accurately in medical records while maintaining compliance with HIPAA regulations.
  • Assist with discharge planning and transition of care from hospital or nursing facility settings to home environments.
  • Conduct triage over the phone or in person to prioritize urgent needs and coordinate appropriate interventions.
  • Support patients with complex needs such as those requiring ventilator management, infusion therapy, or post-acute care services.
  • Maintain infection control standards and ensure all equipment is sterile and properly maintained.
  • Provide education on health coaching topics including geriatrics, pediatric care, behavioral health, or chronic disease management as needed.

Qualifications

  • Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS) are essential.
  • Demonstrated experience in home health nursing with familiarity in medical-surgical procedures and outpatient care settings.
  • Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and patient tracking.
  • Strong knowledge of physiology, anatomy, medical terminology, ICD-9/10 coding, and medication administration protocols.
  • Experience working with diverse populations including seniors, children—including neonatal ventilator management—and individuals with disabilities or developmental disorders.
  • Familiarity with complex care needs such as hospice services, trauma center protocols (Level I & II), ICU experience, or emergency medicine is highly desirable.
  • Ability to perform venipuncture, phlebotomy, sterile processing techniques, and specimen collection/processing competently.
  • Excellent patient assessment skills combined with the ability to educate families on disease management strategies including Alzheimer’s/dementia care and pain management techniques.
  • Knowledge of infection control practices and aseptic techniques to prevent cross-contamination during procedures like suturing or spinal taps.
  • Strong communication skills for effective triage calls and interdisciplinary collaboration; compassionate approach tailored to individual patient needs is essential.
