We're seeking an RN Case Manager who thrives on connection and critical thinking, and is passionate about helping patients live their best possible lives while providing care across Wayne, Oakland, Macomb, Washtenaw, Livingston, and Genesee counties.
In this role, you will:
- Conduct thorough nursing assessments and create individualized care plans.
- Deliver skilled nursing interventions in the home.
- Teach patients and families how to manage health conditions and medications.
- Coordinate with physicians, therapists, and other team members to ensure seamless care.
- Keep documentation accurate, timely, and compliant with state, federal, CHAP, and Joint Commission regulations.
You're a great fit if you:
- Graduated from an accredited school of professional nursing.
- Hold a current Michigan RN license.
- Have at least two years of nursing experience (home health preferred).
- Are compassionate, resourceful, and an excellent communicator.
- Have a valid driver's license, insurance, and reliable transportation.
