Position Overview
We are seeking a highly dependable, field-based SLS Case Manager who will oversee client services, coordinate care, support staff, and ensure quality outcomes.
This is a hands-on leadership role — the Case Manager must be willing and able to step in as a Direct Support Professional (DSP) to maintain continuity of care during staffing gaps or emergencies.
This role combines:
- Case management & care coordination
- Field supervision & quality assurance
- Direct client support when needed
Key ResponsibilitiesCase Management & Care Coordination
- Develop, implement, and monitor individualized support plans (ISP/IPP goals)
- Coordinate services with Regional Centers (e.g., NBRC, ACRC, GGRC)
- Communicate regularly with families, service coordinators, and interdisciplinary teams
- Lead and document care team meetings
- Track client progress and ensure goal advancement
- Provide lunch break coverage for staff at assigned homes
Field-Based Client Support
- Conduct regular in-home visits to ensure quality care and safety
- Provide coaching and guidance to DSP staff in real-time
- Support clients with:
- Activities of Daily Living (ADLs)
- Instrumental Activities of Daily Living (IADLs)
- Community integration and engagement
- Step in to cover shifts as a DSP when staffing gaps occur
- Respond to urgent client needs and on-call situations
Staff Support & Oversight
- Train, mentor, and support DSP staff
- Reinforce professional boundaries and best practices
- Ensure staff are following care plans and documentation standards
- Assist with shift coverage coordination in collaboration with scheduling team
Quality Assurance & Compliance
- Audit and review:
- Shift notes
- MARs (Medication Administration Records)
- Client documentation
- Ensure compliance with:
- Title 17 regulations
- Regional Center requirements
- IHSS standards (when applicable)
- Complete and review Special Incident Reports (SIRs)
- Monitor documentation timeliness and accuracy
Documentation & Reporting
- Maintain accurate, audit-ready client files
- Complete monthly/quarterly reports
- Track outcomes and service delivery metrics
- Utilize agency systems (e.g., AxisCare) for:
- Notes
- Scheduling visibility
- Documentation compliance
Crisis Response & Problem Solving
- Respond to behavioral or environmental crises
- De-escalate situations using trauma-informed approaches
- Ensure client safety and proper incident documentation
- Communicate effectively with leadership during critical incidents
Minimum Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field (preferred)
- OR 3–5 years of experience in SLS, ILS, or similar field
- Experience supporting individuals with developmental disabilities
- Strong understanding of person-centered practices
Required Qualifications
- Valid California Driver’s License (REQUIRED)
- Reliable transportation with insurance
- Willingness to work in the field daily
- Ability to step in and provide direct care (DSP duties) as needed
- Strong documentation and organizational skills
- Ability to pass background clearance (DOJ/FBI)
Preferred Skills
- Knowledge of Title 17 regulations
- Experience working with Regional Centers
- Crisis intervention and behavioral support experience
- Leadership or supervisory experience
- Bilingual (Spanish or other languages a plus)
