Medical Records Assistant – Skilled Nursing Facility (Toluca Lake)

Our skilled nursing facility in the Toluca Lake / North Hollywood area is seeking an experienced Medical Records Assistant to help with our Health Information Management department. What You’ll Do

• Help to oversee all medical records processes to ensure accuracy, confidentiality, and regulatory compliance
• Maintain and audit resident charts, logs, and documentation
• Support clinical teams with timely record completion

What We’re Looking For

• Minimum 2 years of experience as a Medical Records Assistant in an SNF
• Strong knowledge of HIPAA, CMS, and Title 22
• Experience with EHR systems used in long‑term care (PCC, EPIC, etc.)
• Excellent communication and organizational skills
• Experience working with Microsoft Office Suite, copying, faxing, scanning, and uploading documents to various technological platforms/applications
• Must be able to work with minimum supervision, responding to changing priorities and role needs
• Ability to organize and manage multiple tasks
• Able to respond to requests in a fast-paced environment
• Have a minimum High School Diploma or equivalent
