Responsibilities
- Assist elders with daily activities such as mobility, medication reminders, and personal hygiene.
- Provide health education on topics including nutrition, medication management, and disease prevention.
- Support elders in accessing healthcare services, including Medicare and other social programs.
- Conduct data collection related to health status, service needs, and program outcomes for reporting purposes.
- Use motivational interviewing techniques to encourage healthy behaviors and lifestyle changes.
- Facilitate group activities or educational sessions to promote social engagement and mental stimulation.
- Collaborate with healthcare providers, social workers, and family members to develop personalized care plans.
- Maintain accurate records of interactions and services provided while respecting confidentiality standards.
- Advocate for elders’ needs within the community and connect them with appropriate resources.
Skills
- Knowledge of nursing practices, medical terminology, and public health principles.
