What are the responsibilities and job description for the Patient Care Coordinator/Quality Improvement Assistant position at Todos Para La Salud Community Clinic?
SUMMARY
The Patient Care Coordinator works cooperatively with Todos Para La Salud’s providers and personnel to carry out the goals and objectives of the community health clinic, according to the established policies and procedures. They will assist multiple providers in managing the care of patients according to established protocols including, but not limited to: assisting with patient phone calls, referral status updates, medical questions. The Patient Care Coordinator / QI coordinator will attend and actively participate in clinical staff meetings, in services and educational programs. It is the responsibility of the Coordinator to seek out the answers for challenging questions and address issues appropriately, as well to maintain patient confidentiality at all times.
ESSENTIAL DUTIES
- Participate in staff and management meetings as they relate to patient care coordination
- Help implement initiatives to improve patient experience
- Reach out to patients from Gaps in care reports and schedule visit appointments
- Attend training sessions as requested
- Maintain organized QI files, logs, and documentation
- Track and document calls in the patient's chart and notify the care team about the patient's needs
- Ability to work with both clinical and administrative staff
- Able to manage multiple priorities
EDUCATION/EXPERIENCE
1. High School Graduate/GED
2. Minimum 1-2 years of progressive experience in patient care coordination in a healthcare facility, preferably with a federally funded health center doing business in California
3. Current CPR certification or certification within 3 months of initial hire
4. Medical background is necessary to understand the medical terminology, preferred to have at least MA certification but equivalent degree will be accepted
LANGUAGE SKILLS
Knowledge of Spanish language is preferred, but not necessary for this position. If an employee speaks a language other than English, they may be subject to proficiency testing.
