Role Description
Full-Time and Part-Time Service Facilitators in Virginia. The Service Facilitator works directly with individuals and families receiving Medicaid consumer-directed services to help coordinate and manage their in-home care needs. Daily responsibilities include completing in-home visits, maintaining accurate case records, assisting families with understanding their waiver services, helping with caregiver and Employer of Record (EOR) guidance, monitoring Medicaid compliance, and ensuring services are meeting the member’s needs. Service Facilitators also work closely with MCOs, fiscal agents, caregivers, and internal team members to provide consistent support and problem-solving for families. This role requires strong communication, organization, and the ability to build trust with families while managing a caseload effectively.
Qualifications
- Strong communication and interpersonal skills to effectively coordinate with clients, families, and service providers.
- Experience in case management or customer service is a plus.
- Proficiency in organizational and time management skills.
- Familiarity with documentation practices and record-keeping for client services.
- Prior experience in a social services, healthcare, or community-based organization is a plus.
- Basic computer skills, including email, scheduling, and data entry.
- A flexible schedule that can accommodate in-home visits during the day.
