Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
- Administer medications, injections, IV infusions, and other treatments following established protocols and physician orders.
- Assist with medical procedures such as catheterizations, wound suturing, specimen collection/processing, and sterile techniques to ensure patient safety and comfort.
- Maintain accurate and detailed medical documentation within electronic health record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations.
- Provide patient education on medication management, disease processes such as dementia or Alzheimer's care, nutrition, and discharge instructions to empower patients and families.
- Collaborate with interdisciplinary teams including physicians, case managers, therapists, and social workers to coordinate comprehensive care.
- Support triage activities by evaluating patient symptoms in urgent care or emergency room settings and determining appropriate levels of care or referrals.
Qualifications
- Valid LPN license in good standing with current state licensure requirements.
- Proven experience in diverse clinical settings such as critical care (ICU), medical-surgical units, long-term care facilities, assisted living communities, or hospice environments.
- Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation.
- Competence in performing IV insertions, medication administration (including injections), phlebotomy, and catheter care.
- Knowledge of medical terminology, anatomy/physiology principles, ICD coding (ICD-9/10), CPT coding for procedures and treatments.
- Experience working with individuals with disabilities or those requiring specialized dementia or memory care is preferred.
- Ability to perform physical assessments and manage acute pain or airway emergencies confidently.
- Strong communication skills for effective patient education and team collaboration; BLS certification is required.
