Responsibilities
Provide licensed practical nursing care to patients following care plans.
Assess patient needs and document observations accurately for care teams.
Administer medications and monitor for adverse reactions per protocols.
Adhere to regulatory standards and company policies at all times.
Coordinate with multi-disciplinary teams to ensure timely patient progress.
Maintain accurate, up-to-date patient records and eligibility for services.
Meet productivity targets and contribute to pipeline-based performance metrics.
Support quality improvement initiatives to increase patient outcomes.
Educate patients and families on care plans and post-discharge steps.
Maintain a high-visibility work ethic by documenting daily activities and outcomes.
Assist with onboarding and mentoring of new staff to foster growth.
Experience/Qualifications
Valid LPN license in applicable state.
Experience with medication administration and adverse reaction monitoring.
Ability to accurately document observations in electronic health records.
Current CPR/ACLS certification preferred or required.
Familiarity with care plans and regulatory standards (e.g., HIPAA, OSHA).
Strong collaboration with multidisciplinary teams to coordinate care.
Commitment to quality improvement initiatives and patient outcomes.
Demonstrated productivity and contribution to pipeline-based targets.
Excellent patient and family education skills for discharge planning.
Willingness to mentor and onboard new staff to support growth.
On-site availability for contract role with high visibility and daily activity documentation.
