Position Summary
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to serve as Program Director for our Adult Day Health Center. This leadership role oversees daily program operations and ensures the delivery of high-quality medical, social, and supportive services to adults participating in our program.
The ideal candidate has strong leadership skills, experience working with adults in a healthcare setting, and the ability to manage staff while maintaining compliance with healthcare regulations.
Key Responsibilities
- Oversee daily operations of the Adult Day Health Program
- Supervise, train, and evaluate program staff
- Coordinate participant care with healthcare providers and families
- Ensure compliance with state and regulatory requirements
- Conduct chart audits and maintain proper documentation
- Develop program goals, policies, and procedures
- Manage staff schedules, payroll timekeeping, and attendance
- Coordinate admissions and transportation services
- Oversee safety procedures and emergency preparedness
- Support marketing, outreach, and community partnerships
- Monitor program performance and implement improvements
Qualifications
- Active Licensed Practical Nurse (LPN) license required
- Minimum 1 year experience working with impaired adults or seniors
- 2 years of supervisory or management experience preferred
- Experience in healthcare, adult day health, senior services, or similar settings
- Strong leadership, communication, and organizational skills
- Ability to work effectively with a multidisciplinary healthcare team
Requirements
- CPR and First Aid certification
- TB test within the past 12 months
- Recent physical exam (within 12 months)
- Must pass full state background check and driving record check
