About the Position
Join our team as an LPN and provide compassionate, person-centered care to adults and seniors in our Adult Day Program. You’ll work closely with participants, families, nurses, and other team members to support their health, independence, and overall well-being.
What You’ll Do
- Monitor participants’ health and wellness
- Assist with health assessments and nursing evaluations
- Administer medications and treatments as ordered
- Help implement and follow individual care plans
- Provide health education and wellness support
- Communicate with nurses, providers, families, and other care team members
- Document care and changes in participants’ health
- Help maintain a safe, supportive, and caring environment
What You Need
- Graduate of an accredited LPN program
- Current New Jersey LPN license
- Current CPR and First Aid certification
- Good communication and organizational skills
- Caring, dependable, and compassionate approach to care
- Bilingual English/Spanish
