Description
A Licensed Social Worker (LSW) in a skilled nursing facility (SNF) provides comprehensive psychosocial assessments, care planning, and therapeutic interventions to support residents' mental and emotional well-being. They facilitate admissions/discharges, advocate for resident rights, and mediate between families and the interdisciplinary care team.

- Psychosocial Assessments & Care Plans: Conduct comprehensive assessments upon admission and develop individualized, 90-day, or annual care plans.
- Discharge Planning: Coordinate with insurance managers, families, and community resources to manage safe discharges.
- Resident Advocacy & Rights: Ensure compliance with federal/state regulations, promote resident dignity, and protect rights.
- Family/Resident Support: Offer crisis intervention, counseling, and education to help residents adjust to long-term care.

Interdisciplinary Collaboration: Participate in daily rounds, care conferences, and team meetings to review clinical, emotional, and social needs.
Skills, Knowledge and Expertise
Required Qualifications
- Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
- Licensure: Valid state licensure (LSW, LCSW, or equivalent).
- Experience: Previous experience in a nursing home, hospital, or geriatric care is preferred.
- Skills: Strong communication, documentation, and crisis management skills.
