What You’ll Do
- Conduct telehealth physical therapy sessions from home, providing virtual evaluations, follow-up care, patient education, and treatment guidance
- Deliver hands-on, in-person physical therapy care during scheduled onsite visits
- Evaluate patient needs and develop individualized treatment plans focused on improving mobility, strength, safety, function, and overall quality of life
- Collaborate closely with interdisciplinary care teams, physicians, caregivers, and facility staff to ensure coordinated care
- Complete accurate and timely documentation in compliance with company, payer, and regulatory standards
- Monitor patient progress, adjust treatment plans as needed, and advocate for positive patient outcomes
- Utilize technology confidently to create seamless telehealth experiences for patients
- Help support a positive, patient-centered therapy culture rooted in service excellence
- Licensed Physical Therapist (PT) or license eligible in the state of practice
- Graduate of an accredited Physical Therapy Program
- Interest or experience in geriatric rehabilitation or senior living therapy
- Passion for patient-centered care and improving quality of life for older adults
- Excellent communication and interpersonal skills
- Strong time management and organizational abilities
- Comfortable using EMR/documentation systems and technology platforms
- Ability to work independently while remaining highly collaborative
