What are the responsibilities and job description for the Life Enrichment Coordinator position at TLC SUPPORT SERVICES LLC?
Services are individualized and may include, but are not limited to:
• Identifying personal interests, goals, and meaningful activities, both in the home and community
• Duties can include companionship, meal prep, daily activity and ADL assistance(not hands on, directives only).
• Developing an individualized Life Enrichment Plan
• Coordinating social, recreational, educational, and volunteer opportunities
• Connecting clients with community resources and support programs
• Assisting with scheduling appointments and activities
• Coordinating transportation resources
• Researching available benefits and community services
• Assisting with technology that promotes independence and social connection
• Communicating with family members, providers, or support systems (with appropriate authorization)
• Monitoring progress toward identified goals
• Providing ongoing encouragement, accountability, and support
• Making referrals to other professionals or agencies when appropriate
