We are seeking experienced Registered Nurses (RNs) to join a private care team as independent contractors, providing direct bedside care for an independent, active professional with a high-level spinal cord injury (C2-C3, ASIA A) and ventilator dependence. The client is a practicing attorney specializing in health law and policy.
This is a private home health case (not Medicaid or Medicare), requiring skilled nurses with strong clinical judgment, proactive care management, and the ability to deliver high-quality interventions.
- Shifts: 12-hour night shifts 7:00 PM – 7:00 AM and 12-hour day shifts 7:00 AM - 7:00 PM
- Availability: Flexible scheduling; seeking full-time nursing coverage (2–3 shifts per week)
Key Responsibilities:
Direct Bedside Nursing Care:
- Manage ventilator and tracheostomy care, including airway clearance, secretion management, and emergency response.
- Monitor and maintain skin integrity, positioning, and pressure injury prevention for an immobile patient.
- Provide proactive interventions to prevent and manage secondary complications (e.g., autonomic dysreflexia, respiratory infections, hypotension, skin breakdown).
- Perform patient transfers using an electric lift for bed-to-wheelchair transitions.
- Manage bowel and bladder routines, medication administration, and general hygiene assistance.
Clinical Oversight & Proactive Health Management:
- Apply advanced clinical knowledge and critical thinking to anticipate and respond to changing medical needs.
- Collaborate with the Care Coordination Team (Case Manager, Nurse Navigator, Wound Care Nurse) to ensure seamless care.
- Maintain detailed, accurate documentation of clinical observations, interventions, and health trends.
Professionalism & Work Environment:
- Maintain professional conduct at all times (no smoking, personal phone use, or texting during shifts).
- Communicate effectively and respectfully in a professional home and work environment.
- Dress appropriately for a business-professional office setting (when accompanying the client to in-office work).
- Participate in client activities, including professional events, social gatherings, and travel.
- Assist with light cooking and cleaning to maintain a clean and organized care environment.

Required Qualifications:
- Active California Registered Nurse (RN) license in good standing
- Valid California driver’s license and ability to travel as needed
- 3–5 years of hospital or acute care experience (ICU, respiratory, neurology, or complex case management preferred)
- Strong assessment and critical thinking skills with the ability to detect and respond to clinical changes
- Experience with ventilator management, tracheostomy care, and respiratory interventions
- Experience caring for immobile patients or those with high-level spinal cord injuries (or willingness to learn)
- Physical ability to perform:
- Patient transfers using an electric lift
- Turning/repositioning the patient in bed
- Moving quickly during emergencies, bending, reaching, and standing/sitting throughout the shift
Preferred Qualifications:
- Experience in private duty nursing, home healthcare, or long-term acute care
- Familiarity with autonomic dysreflexia, orthostatic hypotension, and other SCI-specific conditions
- Wound care and pressure injury prevention expertise (or willingness to coordinate with Wound Care Nurse)
