SUMMARY:
Drives facility vehicle for resident activities, doctor appointments, shopping, and other scheduled activities.
QUALIFICATIONS:

Education and/or Experience:
High School diploma or equivalent Experience working with seniors is desired
Certificates, Licenses, Registrations:
Must meet the requirements indicated on the Employee Documentation Grid.
Must successfully complete training on listed Required Training Grid and any additional training as required by Federal State and local requirements. Must have a commercial driver’s license and a clean 5-year driving record.
Other Skills and Abilities:
Must pass the Driver Road Test and have an acceptable Motor Vehicle Report each year. (Refer to Drivers' Safety Policy) Understand the needs of the elderly.
WORKING ENVIRONMENT:
Standing and walking through facility during work. Turning, stooping, bending, pushing, pulling and/or lifting while transporting residents and/or in the facility. Works throughout the facility and the vehicle. Some lifting required, 10-15 lb. Must be able to assist residents in and out of the vehicle.
Qualifications:DUTIES AND RESPONSIBILITIES:
- Provide timely and efficient transportation service to residents, according to transportation schedule.
- Must have transportation schedule at all times while performing daily duties.
- Must have transportation schedule at all times while performing daily duties.
- Coordinate the safe return of all residents that use facility transportation, which includes making a list of residents on bus for outings.
- Actively communicates with proper facility staff member to ensure residents' transportation needs are met in a timely manner.
- Return the facility vehicle keys each night to the proper staff member.
- Every Monday will be responsible for completing the Weekly Vehicle Safety Report and Monthly Mileage Report.
- Fully understands and complies with Driver’s Safety Policy.
- Report any improper handling or operation of the vehicle to the Administrator.
- Report any accident or emergency situation to the Administrator as soon as possible.
- Complete an Incident Report for any incident, accident or unusual occurrence to any resident, staff member or visitor which occurred in the facility, or on any transportation event.
- Ensure that the facility vehicle is thoroughly cleaned and vacuumed weekly.
- Ensure the vehicle has proper facility advertisement signs.
- Ensure that the oil in the facility vehicle is changed every 3000 miles.
- Maintain the Transportation Log in the vehicle.
- Assist with implementing the Emergency and Disaster Plan.
- Attend all mandatory meetings and in-service training sessions.
- Perform all duties assigned in an effective, timely and professional manner.
- Observe infection control procedures and properly correct if needed.
- Accept assigned duties in a cooperative manner.
- Report to work on time as scheduled and consistently demonstrate dependability and punctuality.
- Report to work in neat, clean attire and consistently present an appropriate professional appearance according to the uniform policy.
