Titusville Area Hospital is seeking a compassionate, motivated Physical Therapist to provide exceptional care to patients across the lifespan, from pediatrics through geriatrics, in both inpatient and outpatient settings. Position Summary
The Physical Therapist evaluates patients, develops individualized treatment plans, and provides evidence-based physical therapy services to maximize independence, restore function, and improve quality of life. This position works collaboratively with physicians, therapy staff, patients, and caregivers to deliver high-quality patient-centered care.
Essential Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Establish functional, measurable goals based on each patient's needs.
- Provide therapeutic interventions and modify treatment plans based on patient progress.
- Educate patients and caregivers on home exercise programs, treatment plans, and safe mobility techniques.
- Assess the need for assistive devices, splints, and orthotics, and provide recommendations as appropriate.
- Monitor patient response to treatment and communicate with referring providers regarding progress when necessary.
- Complete timely, accurate documentation including evaluations, progress notes, re-evaluations, and discharge summaries.
- Ensure physician orders are received prior to initiating therapy services.
- Accurately document charges for services provided.
- Collaborate with Physical Therapist Assistants (PTAs) and other rehabilitation team members to provide coordinated patient care.
- Participate in department quality improvement initiatives, staff meetings, and continuing education.
- Assist with orientation and clinical supervision of students, volunteers, and new team members as needed.
- Follow all departmental policies, hospital policies, and regulatory requirements.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Pennsylvania Physical Therapist license, or eligibility to obtain licensure (new graduates are encouraged to apply).
- Strong communication, interpersonal, and organizational skills.
- Basic computer proficiency, including electronic documentation.
- Ability to work effectively both independently and as part of a multidisciplinary healthcare team.
- Commitment to providing compassionate, patient-centered care.
- Stroke education (or willingness to complete required training).
