Nurse Practitioner (NP)
A Nurse Practitioner is needed in New Haven, Connecticut.

Salary and Benefits
- Base salary: $105,000 – $135,000 (depending on experience and location)
- Incentive bonus potential: $15,000 – $25,000 annually (based on quality metrics)
- Physician partnership track available
- 6 weeks of total paid time off (includes PTO, holidays, and CME time)
- CME allowance: $5,000 annually
- Tuition reimbursement available

Responsibilities
- Full-time, Monday–Friday schedule (8:00 AM – 5:00 PM)
- Patient volume: 13–16 patients per day
- Visit structure:
- Regular visits – 20 minutes
- Full diagnostic/new patient visits – 40 minutes
- Work within a value-based healthcare model
- Supported by a care team including a scribe, medical assistant, and social worker
- Onsite labs with phlebotomy services and access to RubiconMD for specialty consultations
- EMR systems: Canopy and Greenway
- Participate in community wellness activities (exercise programs, yoga, dance, health events)
- Average of 3–5 providers per clinic (mix of NPs and Primary Care Physicians)
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner certificate program
- Active and unrestricted Nurse Practitioner license in Connecticut
- Active DEA license
- National board certification in one of the following:
- Family Nurse Practitioner (FNP)
- Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
- Adult Nurse Practitioner (ANP)
- Gerontological Nurse Practitioner (GNP)
- Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (preferred)
