We’re hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, schedule stability, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance.
You’ll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Plymouth, Rockland, and Sandwich. You’ll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care.

• Manageable census of approximately 20 patients
• Additional compensation for rotating weekend on-call coverage

What makes you a great fit
• You hold an active Massachusetts RN license
• You have at least one year of nursing experience
• Hospice, home health, ICU, or oncology experience is preferred

• You’re comfortable independently managing a field-based caseload
• You work well as part of an interdisciplinary healthcare team
• You’re comfortable traveling throughout Plymouth, Rockland, and Sandwich
Compensation & Benefits
• Base salary: $104,000–$114,400 annually
• Weekend on-call: $600 per weekend
• Routine on-call visits: Additional $100 per visit
• On-call admissions: Additional $150 per admission
