The Reentry Family Service Coordinator provides psychiatric rehabilitation services and intensive reentry support to individuals returning to the community after long-term incarceration under the Juvenile Restoration/Second Look Act and related sentencing reform initiatives. Many program participants have been incarcerated for 30–50 years and require comprehensive support to successfully reintegrate into society.This position delivers community-based PRP services, helping participants develop the skills, resources, and supports necessary to achieve stability in housing, employment, mental health recovery, community integration, and independent living.The coordinator serves as both a rehabilitative case manager and reentry support specialist, ensuring individuals receive structured programming, skill development, and coordinated services to reduce recidivism and promote long-term stability.Key ResponsibilitiesPsychiatric Rehabilitation Services (PRP)Provide individual and group psychiatric rehabilitation services in accordance with Maryland PRP regulations.Assist participants in developing individualized rehabilitation plans (IRPs) that focus on recovery, skill development, and community integration.Support individuals in developing coping skills, daily living skills, and emotional regulation strategies.Provide psychoeducation and structured skill-building sessions related to mental health recovery and independent functioning.Document all services in the electronic health record in compliance with PRP billing and documentation standards.Monitor participant progress and adjust rehabilitation goals as needed in collaboration with the treatment team.Reentry Support ServicesProvide specialized support to individuals transitioning from long-term incarceration to community living.Assist participants in navigating modern technology, financial systems, transportation, and digital literacy.Help individuals secure essential resources including:Identification documentsSocial Security benefitsHealthcare enrollmentHousingEmployment opportunitiesSupport individuals in rebuilding family and community relationships where appropriate.Provide guidance and mentorship to address the psychological and social challenges associated with long-term incarceration.Care CoordinationCoordinate services with behavioral health providers, housing programs, employment programs, and community organizations.Maintain communication with probation/parole officers, legal representatives, and reentry partners when applicable.Connect participants to education, job training, and workforce development programs.Advocate for participant access to services and resources within the community.Group FacilitationFacilitate structured reentry and life-skills groups focused on:Life skills and daily functioningEmployment readinessTechnology and financial literacyEmotional adjustment after incarcerationCommunity reintegrationSupport peer-led recovery and reentry groups.Documentation and ComplianceComplete timely and accurate documentation including:Progress notesRehabilitation plansMonthly service summariesOutcome trackingMaintain compliance with COMAR PRP standards, Medicaid requirements, and organizational policies.Participate in quality assurance reviews, audits, and CARF accreditation processes.Additional ResponsibilitiesParticipate in treatment team meetings and case conferences.Conduct community-based visits and outreach when appropriate.Support crisis intervention and stabilization when participants experience behavioral health challenges.Assist with program activities, workshops, and reentry programming initiatives.Minimum QualificationsMinimum 1–2 years experience in behavioral health, case management, PRP services, or reentry programming.Experience working with justice-involved populations strongly preferred.Knowledge of community resources and social service systems.Preferred QualificationsExperience providing Psychiatric Rehabilitation Program (PRP) services.Knowledge of trauma-informed care and recovery-oriented services.Familiarity with reentry barriers faced by individuals returning after long-term incarceration.Required SkillsStrong interpersonal and relationship-building skills.Ability to work with individuals experiencing mental health challenges and complex trauma.Excellent documentation and organizational skills.Cultural competency and ability to work with diverse populations.Ability to maintain professional boundaries while providing supportive mentorship.Work EnvironmentCombination of office-based, community-based, and programmatic work.May require travel within the Baltimore region to support participants in the community.Company DescriptionTIME ORGANIZATION HAS SERVICED THE COMMUNITY FOR OVER 25 YEARS!WE ARE FULL-SERVICE OUTPATIENT MENTAL HEALTH CLINIC THAT PROVIDES INDIVIDUAL, GROUP AND FAMILY THERAPY, AS WELL AS, MEDICATION MANAGEMENT FOR YOUTH AND ADULTS.
