Position Summary
The Clinical Care Coordination Manager prevents the need for hospitalization of patients through education. Educate patients on the most appropriate place to receive care, at the most appropriate time.
Major Duties and Responsibilities
- Responsible for overseeing the Shared Savings Programs.
- Prepare patient and staff teaching material & oversee the teaching of self-care management.
- Oversite of nurses, and medical assistants.
- Responsible for hiring the best candidates who are qualified, motivated and, disciplined.
- Responsible for preparing and updating GAP Training and materials.
- Primary point of contact responsible for updating hospitals when new physicians join TPMG.

Education / Training / Requirements
- Bachelor's Degree in Health Administration, Business, or related field;
- Master of Health Administration (MHA) Degree preferred;
- One (1) to Three (3) years of related experience / training.
- Registered Nurse / Nurse Practitioner licensure.
