Job Summary

Key Responsibilities
- Conduct intake interviews and assessments to determine client needs.
- Connect clients with appropriate community resources, services, and benefits.
- Maintain accurate and timely documentation of client interactions and progress.
- Advocate on behalf of clients with service providers and other stakeholders.
- Monitor and evaluate client progress and adjust service plans as needed.
- Collaborate with social workers, healthcare providers, and legal representatives.
- Ensure compliance with all relevant policies, procedures, and regulations.
- Previous experience in case management or social services is highly desirable.
- Strong interpersonal and communication skills.
- Knowledge of local community resources and support services.
- Commitment to confidentiality and ethical standards.
- Bilingual: English and local language
- Crisis intervention training.
- Familiarity with trauma-informed care practices.
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s preferred).
