The Opportunity
We are currently hiring a Community Support Worker (Direct Support Professional / DSP) to provide Nebraska Medicaid Community Support services (H2015) to individuals living with serious mental illness in the Omaha, Nebraska area. This position begins as part-time with an afternoon schedule, with a clear path to full-time as our program grows.
This is a field-based role — you will work directly with clients in their homes and in the community, helping them build the practical skills and supports they need to thrive.
What You Will Do
As a Community Support Worker at Thrivion Mental Wellness, you will:
- Provide direct, in-home and community-based rehabilitation and skill-building support to individuals with serious and persistent mental illness (SPMI), including those with co-occurring developmental disabilities or substance use disorders
- Support clients in developing and maintaining activities of daily living including hygiene, meal planning, household management, and medication adherence
- Assist with budgeting, financial management, and navigation of community resources
- Accompany clients to medical, psychiatric, and community appointments to support engagement and follow-through
- Provide relapse prevention support and assist clients in identifying early warning signs and coping strategies
- Assist clients in accessing and coordinating services including mental health care, primary care, housing supports, transportation, and benefits navigation (Medicaid, SSI, SSDI, AABD)
- Implement individualized treatment plan goals developed in collaboration with the clinical supervisor and client
- Document services provided accurately and in compliance with Nebraska Medicaid and Joint Commission standards
- Participate in 30-day clinical supervision reviews and ongoing team communication
- Support crisis planning and assist clients in utilizing their crisis and relapse prevention plans
- Work as part of a collaborative, clinically supervised team to ensure coordinated, person-centered care

Pay: $27.00 - $30.00 per hour
