Community Outreach Worker

Position Summary

The Outreach Worker plays a critical role in connecting individuals experiencing homelessness, behavioral health challenges, or recovery needs to culturally responsive services and stable housing. This position focuses on outreach to Chuukese community members and requires fluency in both Chuukese and English. The Community Outreach Worker will build trust, provide navigation support, conduct field-based engagement, and strengthen connections between participants and community partners, including behavioral health providers, housing programs, and culturally specific organizations.

Key Responsibilities

Outreach & Engagement

• Conduct field-based outreach in shelters, encampments, community spaces, on the street, and service locations.
• Engage Chuukese individuals using culturally appropriate communication and trauma-informed practices.
• Provide information about recovery housing, behavioral health services, and community resources.
• Build trusting relationships that support long-term engagement and stability.
• Participation in community wellness events, which occasionally may require working on a Saturday.

Navigation & Support

• Assist participants in accessing treatment through partners such as Lifeline Connections, Rainier Springs, Sea Mar, Columbia River Mental Health, WPIHB, PICA-WA, VA, and CCVAC.
• Coordinate participant appointments and provide transportation as needed to ensure warm, supported handoffs.
• Help individuals obtain identification, benefits, medical coverage, and other essential documents.
• Provide culturally grounded support to reduce barriers and increase service participation.Housing Coordination
• Assist individuals in completing housing applications and preparing for placement in safe, stable housing or into Thrive2Survive recovery homes, as appropriate.
• Conduct initial needs assessments and support transition planning.
• Maintain regular contact with residents to ensure ongoing stability and engagement.

Community Collaboration

• Work closely with culturally specific organizations and behavioral health providers.
• Participate in team meetings and case consultations.
• Strengthen partnerships that improve access to culturally responsive and population-specific services.
• Work with other local outreach teams.

Documentation & Reporting

• Maintain accurate, timely case notes and data in accordance with grant requirements.
• Track outreach contacts, referrals, service engagement, and participant outcomes.
• Ensure documentation meets standards for confidentiality, compliance, and quality assurance.

Required Qualifications

• Knowledge of homelessness, behavioral health, substance use recovery, or social services.
• Ability to build rapport with individuals experiencing trauma, crisis, or instability.
• Strong communication, problem-solving, and cultural humility.
• Ability to work independently and in field-based environments.
• Valid driver’s license and reliable transportation.

Preferred Qualifications

• bilingual individuals who can speak, read, and write in one of the following languages; Chuukese, Spanish, Russian, or Ukrainian
• Experience working with Chuukese communities or other Pacific Islander populations.
• Experience in outreach, case management, peer support, or community health work.
• Familiarity with Clark County’s behavioral health and housing systems.
• Lived experience in addition/recovery, homelessness, re-entry, or mental health (highly valued but not required).
• Training in trauma-informed care, harm reduction, or motivational interviewing.
• Peer Support certification – or ability to obtain within first 6 months.
• Recovery Coach certification - or ability to obtain within first 6 months.

Work Environment

• Field-based work in community settings, encampments, Safe Stay / Safe Park sites, shelters, unsheltered locations, and partner agencies.
• Office-based documentation, team meetings, and ongoing case management and one-on-one client meetings.
• Regular interaction with individuals experiencing behavioral health challenges, substance use, or housing instability.
