We aim to create a thriving, empowered community where everyone has access to stable housing and the support needed to rebuild their lives.
Our on-site team will connect residents to community resources to address struggles that impact their quality of life. These resources will address issues such as:
- Food insecurities
- Substance abuse and dependency
- Harm reduction
- Mental health
- Connection to medical and dental providers
- Income stabilization
- Mediation in communal living situations
- Building community supports
- Behavioral and safety planning
In addition to connecting the residents to existing community resources, our staff will also be bringing resources on-site for easier access to residents with transportation barriers, as well as hosting their support groups and classes.
This position will be split between our Ukiah and Clearlake properties.
Your role:
- Coordination of services for program participants to ensure that participants are receiving essential services to increase housing stability.
- Processing requests for housing-related financial assistance, including assessments, program enrollment documentation, and check requests.
- Creates a housing stability plan, with participants, that includes housing, income, and other goals to help move participants to self-sufficiency.
- Provide case management services targeting maintaining housing.
- Meet with participants regularly to assess progress toward housing stability goals
- Attend program meetings with internal and external partners to coordinate services and ensure quality services.
- Responsible for complete documentation as required by all program components. Case files need to be complete, accurate, and organized according to Abode standards. Files/cases should be closed in accordance to program standards.
Qualifications:
- 2-3 years of experience in case management and strong knowledge of community resources.
- High School Diploma/GED required. AA preferred
- Reliable transportation along with a valid and current CA Driver's License and insurance is required.
Pay: $20.00 - $22.00 per hour
