Full-Time: Monday-Friday 8am-5pm
SUMMARY
The LPN is responsible for monitoring the treatment needs of patients and making sure they are met in accordance with agency policies, procedures, state and federal guidelines and regulations. The LPN will work with clinical staff, administrative staff and medical QHP, staff to cover the needs of mental health and substance abuse services, within the scope of an LPN license.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Responsible for adhering to standards of care to ensure program patients receive preventative, routine, ongoing, follow-up and emergency medical services.
- On a daily basis the LPN is responsible for delivering optimal care by:
- Working directly with Qualified Medical Provider (QMP) to ensure high quality of care is delivered to patients
- Working directly with QMPs to prepare patient charts, medication reconciliation, and all other duties assigned to support efficient and effective care delivery in the outpatient clinic.
- Coordinating the medical and behavioral health needs of program patients.
- Monitoring the health and well-being of patients via obtaining vitals, height and weight, and EKGs at the request of the NPs.
- Accurately documenting all medical services provided to patients per policy.
- Communicating with medical service providers, patient emergency contacts, and program staff, and then documenting the communication.
- Managing and responding to medication calls from patients and pharmacies to provide timely care.
- Complete Prior Authorizations, mostly using CoverMyMeds.com when necessary.
- Maintaining the clinical chart in accordance with THRIVE policy and procedure, OASAS regulation, HIPAA, and 42 CFR.
- Reporting any and all medication or documentation errors, as outlined in THRIVE policy and procedure, to a program Director or his/her designee.
- Ensuring all prescribed injectable medications are administered and recorded in compliance with THRIVE policies and procedures.
- Administer urine analysis and breathalyzer screenings as requested by the prescribers
- If assigned, designee duties, call in refill orders for prescriptions.
- Maintain the Nursing “station” to meet program needs.
- Provide support as needed for medical related services to medical providers, i.e.. MD, NPP
- Collect collateral information, including HealtheConnections, PSYCKES, to collaborate with other healthcare providers, share vital data and add value to ensure quality patient care and improved health outcomes.
- May be delegated to assist with Health Coordinator and the Training Coordinator duties to ensure the provision of education, risk reduction, counseling and referral services to all patients regarding HIV and AIDS, tuberculosis, hepatitis, sexually transmitted diseases and other communicable diseases.
- Maintain standards of care as described in Principles of Practice, protocols and Scope of Practice as approved by the State of New York Board of Nursing and the American Medical Association.
- Provide emergency first-aid
- Practice safety, environmental and infection control methods
- Interact with community resources regarding health-related issues.
- Be sensitive and responsible to public relations within the community
- Participate in clinical supervision at a minimum bi-weekly
- Attend staff meetings, in-service and conference/trainings as assigned
- Will be required to perform other duties as requested, directed or assigned.

EDUCATION and/or EXPERIENCE
Required: Must be licensed as a Licensed Practical Nurse in New York State; support the goal of a person-centered approach, knowledge of addiction, and must subscribe to a Professional Canon of Ethics.
Preferred: Experience in the Chemical Dependency and/or Mental Health field
