Thrive Therapy of Kansas is seeking a compassionate, organized, and dependable Case Manager to join our growing team. This role is ideal for someone who enjoys working directly with clients, coordinating services, solving problems, and helping individuals with brain injuries access the support they need to be successful in their homes and communities.
Thrive Therapy provides services to individuals served through the Kansas Brain Injury Waiver. Our team works with clients in their homes, in the community, and in our therapy centers to help them build independence, improve quality of life, and reach meaningful goals.
About the Role
The Case Manager is responsible for coordinating care and services for assigned clients, communicating with clients and families, supporting therapy teams, completing required documentation, and helping ensure each client receives services that are appropriate, consistent, and person-centered.
This position requires strong communication skills, attention to detail, follow-through, and the ability to manage multiple client needs at one time.
Responsibilities
Responsibilities may include:
- Coordinate services for clients receiving Brain Injury Waiver services
- Communicate regularly with clients, families, guardians, caregivers, therapists, and other team members
- Complete client visits, check-ins, and follow-up contacts as required
- Assist with care coordination, service planning, and problem solving
- Monitor client progress, needs, concerns, and changes in condition
- Help identify barriers to services and work with the team to resolve them
- Maintain accurate and timely documentation
- Participate in team meetings, staffing discussions, and client care planning
- Communicate concerns to leadership when client needs, safety concerns, or service issues arise
- Support clients in accessing community resources and services
- Maintain confidentiality and follow all company policies and regulatory requirements

Qualifications
Preferred qualifications include:
- Experience in case management, social services, healthcare, behavioral health, home and community-based services, or a related field
- Experience working with individuals with disabilities, brain injuries, mental health needs, or complex medical/social needs preferred
- Strong written and verbal communication skills
- Ability to complete accurate and timely documentation
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel locally to meet with clients as needed
- Bachelor’s degree in social work, psychology, human services, healthcare, or a related field preferred, but relevant experience may be considered
