About the Program

The HEAL Trauma Program (“Healing and Empowering All Living with Trauma”) is part of a series of community-based initiatives funded by the Children’s Services Council of Broward County (CSC). These programs help youth and families in targeted communities—such as Pompano Beach, Deerfield Beach, and Central East Fort Lauderdale—address and heal from trauma through coordinated care, advocacy, and access to resources.

Thrive and Success Community Outreach, Inc. is proud to partner with CSC to provide trauma-informed, family-centered support through our Community Mental Health Workers (CMHWs). Our CMHWs work directly with families in their homes and communities to reduce barriers, build resilience, and strengthen connections to mental health, education, and employment opportunities.

Program Goals

  • Address community trauma and promote healing and resilience among youth and families.
  • Connect families to essential community resources and support systems.
  • Provide direct care, resource navigation, and advocacy for families affected by trauma.

Services Provided

  • Financial Assistance: Help with rental payments, utilities, and groceries during emergencies.
  • Childcare Support: Assist families in finding affordable, out-of-school care options.
  • Enrichment Activities: Coordinate workshops such as yoga, arts, and music for self-expression.
  • Community Support: Offer trauma-informed care and navigation through the social service system.
  • Rapid Response: Provide immediate referrals and resources after traumatic events.

Position Overview

As a Community Mental Health Worker (CMHW), you will provide in-home and community-based support to children and families. This position focuses on care coordination, advocacy, and resource navigation to improve access to mental health care and community services. The ideal candidate is compassionate, motivated, and passionate about helping families heal and thrive.

Key Responsibilities

  • Conduct community outreach and engagement to connect families with mental health and social services.
  • Provide care coordination, advocacy, and referrals to appropriate support resources.
  • Conduct needs assessments and create individualized service plans.
  • Facilitate informal counseling, crisis response, and brief interventions as needed.
  • Support community events and wellness activities that promote emotional and social well-being.
  • Maintain accurate case documentation and collaborate with interdisciplinary treatment teams.
  • Participate in regular supervision and team meetings to support continuous learning and quality care.

Qualifications

  • High school diploma or GED required; some college in human services preferred.
  • Valid Driver’s License and reliable transportation required (field-based role).
  • Bilingual (English/Spanish or English/Creole) preferred, but not required.
  • Entry-level experience in behavioral health, community outreach, or social services is welcomed.
  • Must complete a 30-hour CMHW training (paid) and 500-hour practicum (paid) prior to independent field work.
