Medical Records Clerk
Position Summary
The Medical Records Clerk performs routine and complex clerical work supporting the medical records function at a Thrive Medical Services correctional site. This role maintains complete, accurate, and confidential patient health information; manages records requests; and supports the clinical team by ensuring records are organized, compliant, and readily available. Work is performed under close supervision according to established procedures.
Essential Duties and Responsibilities
Records Management
- Compile, organize, and maintain patient medical records to document condition and treatment.
- Review records for completeness, accuracy, and compliance with policy and regulations.
- Scan and index progress notes, lab results, evaluations, and medication lists into the electronic medical record.
- Convert and accurately enter data from paper charts into the electronic medical record.
- Thin and reorganize charts as needed in accordance with records department policy.
Information Requests and Processing
- Manage incoming and outgoing records requests and release information to authorized persons or agencies per regulations.
- Process patient admission, transfer, and discharge documents.
- Assist patients in completing release of information authorizations and related forms.
- Process and prepare business or government forms as required.
Confidentiality and Compliance
- Protect the security of medical records and maintain strict confidentiality at all times.
- Transport and handle records in compliance with HIPAA regulations.
- Follow facility safety and security policies, including procedures for keys, sharps, and secure areas.
- Adhere to Thrive Medical Services policies, NCCHC standards, and applicable laws.
Qualifications
- High school diploma or GED equivalent required.
- Two (2) years of medical records experience, or an equivalent combination of education, training, and experience.
- Working knowledge of electronic medical records and health record indexing/retrieval systems.
- Strong data entry, organizational, and active-listening skills with attention to detail.
- Ability to maintain confidentiality and work effectively in a secure correctional environment.
Preferred Qualifications
- Prior correctional or detention healthcare records experience.
- Familiarity with HIPAA and NCCHC documentation standards.
