Thrive Home Health is a start-up agency in Meadville, PA seeking a PRN Physical Therapist to join our team starting at $95 per Evaluation Visit. Higher compensation for OASIS Visits.
Responsibilities
- The Physical Therapist consults, evaluates, plans and administers skilled Physical Therapy services prescribed by a physician to homebound patients (18 years of age and older) to restore function, relieve pain and prevent disability following illness, disease, injury, or surgery
- Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved
- Interacts with any and all members of the care group both internal and external to the organization as needed
Qualifications
- Overview: Must be a licensed and or a registered Physical Therapist
- Graduate of an accredited university with a B.S., Doctorate, MA, or MS in Physical Therapy that the APTA recognizes
- PT holds a current license and/or registration as a Physical Therapist in the state of Pennsylvania
- Preferred: Previous home health experience
