The following is required in order to qualify for a RN or LPN Supervisor position with Thrive
Healthcare, LLC:
Be licensed as a Registered Nurse or Licensed Practical Nurse in the state of GA
Positive previous work experience verified by providing a 5-year employment history/ or
complete employment history if person has not been employed 5 years
At least 5 years of experience as a nurse
At least one-year experience in a nurse managerial role
At least one-year experience in home health
Pass a criminal background check and drug screening
Have no history of abuse or neglect
Pass a TB Skin test
Advance interpersonal skills
Excellent organizational and planning skills
Registered Nurse (RN)/ Licensed Practical Nurse (LPN) Supervisor: Mid- management personnel
who are responsible for the following PRN tasks:
Registered Nurse (RN) Supervisor- Mid-management personnel who is responsible for the
following PRN responsibilities:
Conduct initial visit and routine supervisory visits (reassessments) for all clients as approved by (HFRD/ DCH)
Conduct client reassessments every 60 days.
Facilitate PCA Training, in-home/ one-on-one client trainings, Proxy Caregiver Trainings an
Conduct Annual Observed Competency checks as a part of the Annual Employee Evaluation
process.
Facilitate Proxy Caregiver Training as needed.
Review and sign off on client visit documentation.
Conduct follow up meetings with staff, clients, client responsible party as needed and document
interactions with Nurse Progress Notes (via form or email version)
