The following is required in order to qualify for a RN or LPN Supervisor position with Thrive
Healthcare, LLC:
 Be licensed as a Registered Nurse or Licensed Practical Nurse in the state of GA
 Positive previous work experience verified by providing a 5-year employment history/ or
complete employment history if person has not been employed 5 years
 At least 5 years of experience as a nurse
 At least one-year experience in a nurse managerial role
 At least one-year experience in home health
 Pass a criminal background check and drug screening
 Have no history of abuse or neglect
 Pass a TB Skin test
 Advance interpersonal skills
 Excellent organizational and planning skills

Registered Nurse (RN)/ Licensed Practical Nurse (LPN) Supervisor: Mid- management personnel
who are responsible for the following PRN tasks:
 Registered Nurse (RN) Supervisor- Mid-management personnel who is responsible for the
following PRN responsibilities:
 Conduct initial visit and routine supervisory visits (reassessments) for all clients as approved by (HFRD/ DCH)
 Conduct client reassessments every 60 days.
 Facilitate PCA Training, in-home/ one-on-one client trainings, Proxy Caregiver Trainings an
 Conduct Annual Observed Competency checks as a part of the Annual Employee Evaluation
process.
 Facilitate Proxy Caregiver Training as needed.
 Review and sign off on client visit documentation.
 Conduct follow up meetings with staff, clients, client responsible party as needed and document
interactions with Nurse Progress Notes (via form or email version)
