The Role:

The Manager of Intake & Case Management is a key leadership role overseeing Thrive’s Access and Case Management teams. This position ensures timely and effective service linkage for behavioral health clients by managing intake processes, referral pathways, and transitions across programs. The Manager also plays a critical role in team supervision, program development, and strategic initiatives, including the expansion of services such as Community Support for Individuals (CSI), Community Support Teams (CST), or Assertive Community Treatment (ACT).

Key Responsibilities:

· Supervise the Access and Case Management team to support client intake, service connection, and care coordination

· Monitor workflows to ensure timely, equitable access to services

· Partner with the Access Coordinator to manage staffing capacity and service availability

· Lead team meetings, provide individual supervision, and complete staff evaluations

· Support smooth client transitions between internal programs and departments

· Oversee referrals and ensure timely follow-up for case management services

· Represent the team in internal meetings, community events, and regional coalitions

· Respond to service inquiries and assess complex or high-need client cases

· Complete intake assessments for clients joining group-based services

· Assist in developing new case management programs (e.g., CSI, CST, or ACT models)

· Use service data and trends to guide program planning and staffing

· Maintain systems to track referrals, waitlists, and unmet service needs

· Use client feedback to improve access and service workflows

· Ensure compliance with licensing, ethical, and funding requirements

Qualifications:

  • Master’s degree in Social Work, Counseling, or a related behavioral health field.
  • Clinical licensure in Illinois (LCSW, LCPC, or equivalent), or eligibility to obtain licensure within 6 months.
  • Experience in behavioral health settings with a focus on access, case management, or clinical program leadership.
  • Demonstrated knowledge of behavioral health systems and care models including CSI, CST, or ACT.

Preferred:

  • Experience in launching or expanding clinical or case management programs.
  • Experience supervising a diverse team of clinical and non-clinical staff.
  • Bilingual or multilingual abilities.

Schedule:

  • Flexible days and hours
  • Must work 5 days per week
  • May work remotely 1-2 days per week

Salary : $62,000 - $65,000
