Position Summary
The Support Coordinator is a vital member of Thrive’s Care Management Program and is responsible for coordinating services and supports for adults with developmental disabilities. This role involves managing an assigned caseload, maintaining comprehensive oversight of service delivery, and ensuring that each individual’s supports align with their personal goals, preferences, and needs.
The Support Coordinator works closely with individuals, families, service providers, and community partners to develop and monitor individualized, person-centered service plans. The position requires regular in-person visits and ongoing phone-based engagement, as well as flexibility to accommodate client schedules and evolving circumstances.
Key Responsibilities
- Develop and maintain individualized, person-centered service plans.
- Assist individuals and families in navigating community resources, benefits, and available supports.
- Conduct required quarterly home visits and monthly follow-up contacts.
- Monitor service delivery to ensure quality, effectiveness, and regulatory compliance.
- Maintain timely, accurate, and thorough documentation and case records.
- Collaborate with internal team members and external service providers to address changes in needs, services, or goals.
- Ensure compliance with all New Jersey Division of Developmental Disabilities (DDD) regulations, Medicaid requirements, and agency policies.
- Advocate for individuals to promote choice, independence, and community inclusion.
Qualifications
- Bachelor’s degree or higher in Social Work or a related human services field.
- Minimum of one (1) year of case management experience.
- Minimum of one (1) year of experience supporting individuals with developmental disabilities.
- Experience may include paid employment, volunteer service, internships, or caregiving roles.
- Experience coordinating services for mixed populations that included individuals with developmental disabilities will be considered.
- Valid New Jersey driver’s license, reliable personal transportation, and willingness to travel locally for visits.
- Strong organizational skills, ability to work independently, and excellent attention to detail.
- Proficiency with Microsoft Office and Adobe Acrobat Pro.
- Ability to maintain a functional home workspace, including a laptop, printer, cell phone, reliable internet access, and hotspot.

Job Types: Full-time, Part-time
Pay: $49,270.21 - $59,336.18 per year
