CASE MANAGER

ADULT OUTPATIENT PROGRAM

FULL TIME – NON EXEMPT

General Summary: Provides direct care, support, education and advocacy to clients, their family members and caregivers.  Identifies clients’ needs and barriers affecting their mental health, physical health and wellbeing.  Partners with clients to assess treatment needs, collaborate on developing a person-centered recovery plan, and review progress.  This position is community and office based and may be required to provide transportation as needed. 

 

Schedule: Mon-Fri, First Shift

Location: Warwick, RI

Essential Responsibilities:

  • Provide screening, assessment, education, treatment, and follow-up services to clients on issues concerning mental health, physical health, substance use disorder, vocational needs, and living skills.
  • Conduct needs assessments and works with the assigned Care Coordinator to ensure all client’s needs are being met.
  • Collaborate with other providers, agencies, and individuals in the client’s network of care.
  • Empower and educate clients on building skills to manage activities of daily living independently.
  • Monitor signs and symptoms of psychiatric functioning, monitor medication regimen and document findings. Reports any variance to appropriate clinical supervisory staff.
  • Educate clients, their family members and caregivers on mental illness and/or substance abuse disorder as appropriate.
  • Educate clients on effective techniques for recognizing and managing symptoms and assist with medication management when needed.
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives.
  • Provide assistance with living arrangements and income maintenance to ensure safe and affordable housing.
  • Provide assistance and education in the management of client rep payee accounts.
  • If appropriate for assigned program, responsible for ensuring the client has an active recovery plan and is continuously working towards goals and objectives, following all accreditation, state and federal requirements.
  • Enters all necessary information in the client record such as progress notes, billing information and data in a timely manner and in accordance with Thrive standards. 
  • Schedule and maintain client hours sufficient to achieve individual / program billing targets.
  • Participate in clinical team meetings to ensure smooth coordination of client services and treatment.
  • Identify when crisis intervention is needed and triage to appropriate team. 
  • Accompany client to community appointments, provide advocacy as needed.
  • Assist in coordination and support of client’s physical healthcare and needs.
  • Carry and enliven a positive Thrive image consistent with the Thrive’s Guiding Principles and professional code of ethics.
Qualifications:

Education, Experience and Competencies:

  • Bachelor’s Degree in the Human Service field or related field required.
  • Minimum two (2) years experience in Human Service field working with clients with disabilities preferred.
  • An equivalent combination of education and experience could be considered.
  • Training and experience in Motivational Interviewing and Trauma Informed Care preferred.
  • Experience or training in related state and federal assistance programs preferred.
  • Basic computer literacy skills and familiarity with an electronic health record are necessary.

 

Certifications, Licenses Requirements:

  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required.

All employees are considered benefit eligible if their schedule is 17.5 hours per week or more. Time-off balances are pro-rated for part-time employees. Insurance plans remain the same regardless of FT or PT benefit eligibility.
