What are the responsibilities and job description for the Service Coordinator position at Thrive Alliance?
The Service Coordinator assists families with a child aged birth to 3 years of age with a disability in accessing a community-based, comprehensive system of services. These services are individually tailored to meet the families' needs and are provided in the child's natural environment. The Service Coordinator coordinates ongoing eligibility and the Individualized Family Service Plan (IFSP), serves as the family advocate, and is the initial resource for Early Intervention Services under Part C of IDEA.
The Service Coordinator will conduct in-home assessments, lead the IFSP and teams which include, but are not limited to, the family, medical providers, Eligibility Determination Team, educational providers, and other community-based resources, through the process of problem-solving, negotiation, and resolution of conflicts to provide the best options available for the child and family.
