What are the responsibilities and job description for the Housing CM position at Thrive Alabama?
Performs detailed case management services for individuals and families in need of homelessness prevention and rapid re-housing assistance.
Duties and Responsibilities
1. Complete intake and assessments to ensure all program participants meet enrollment requirements by verifying eligibility, housing status and income.
2. Complete all documentation to comply with requirements from the Alabama Department of Economic and Community Affairs (ADECA) and the Department of Housing and Urban Development (HUD).
3. Conduct monthly appointments to assist participants in achieving identified goals that are tenants in the Housing Programs (ARAP, & TBRA) according to the amount of required monthly encounters.
4. Complete all Short-Term HOPWA (STRMU) Applications that are referred by a Clinic Social Worker to assist client’s housing needs.
5. Assist the Primary Care clients with finding affordable housing options and referrals to organizations for financial assistance to assist with emergency housing needs.
6. Perform housing searches, inspections, and rent reasonableness evaluations.
7. Participate as the agency representative on NACH Community Resource Committee that meets monthly.
8. Collaborate with other agencies or organizations to assist program participants in meeting their needs and building self-reliance.
9. Prepare and make presentations to local community groups.
10. Attend case management meetings, agency staff meetings, trainings, city council meetings, and other related events.
11. Provide referrals to other community resources, as needed.
12. Complete daily documentation in both the electronic health record (eClinicWorks).
13. Submit monthly documentation and reports, including billing sheets, to the Director of Case Management Services, Chief Financial Officer, and Chief Executive Officer.
14. Process referrals from Social Work staff to determine grant eligibility for clients. (HOPWA, RW EFA, ARAP or ALEG)
15. Enter required client data into HMIS (Access) database for the required grants and housing programs.
16. All other duties as assigned.
Knowledge, Skills, and Abilities
1. Knowledge of the concepts and principles of case management.
2. Ability to operate a vehicle and willingness to travel to neighboring counties on a weekly basis.
3. Ability to interact effectively with clients and families from diverse populations.
4. Strong desire to work with homeless populations as well as transient individuals and families.
5. Ability to establish and maintain effective team relationships with other community organizations and the public.

Minimum Qualifications
Bachelor's degree in social work (or other related field) or at least three years of case management experience.
