Why This Role Matters

Washington State’s Wraparound with Intensive Services (WISe) is a community-based program grounded in System of Care (SOC) values, designed to provide comprehensive behavioral health services to Medicaid-eligible youth aged 20 and under with complex mental health needs, along with their families. WISe emphasizes family-driven, youth-guided care that is culturally and linguistically responsive, aiming to help youth thrive at home and in their communities while reducing costly out-of-home placements.

As a vital member of the WISe team at CHSW, you will coordinate and facilitate the wraparound process, ensuring that individualized care plans are developed and implemented effectively. Your role involves leading care coordination efforts, facilitating team meetings, and tracking progress through each phase of WISe to support positive outcomes for youth and families. This position requires handling protected health information (PHI) with strict confidentiality and professionalism to uphold the highest standards of care and compliance.

Job Duties

  • Prepares for Child and Family Team (CFT) meetings by collaborating with team members to develop the agenda, schedule meetings, invite all participants, and create visual aids or tools to support the meeting process.
  • Facilitates CFT meetings, by guiding team discussions, fostering collaborative decision making, and serving as the central communication point for the team and families.
  • Proactively contact absent team members before meetings together their input and ensure their perspectives are included.
  • Cultivate an effective, inclusive team culture, engaging all members and identifying their needs to meet agency and program mandates.
  • Strengthen “natural supports” by understanding family history, culture, and community resources, helping you and families connect with supportive individuals and networks.
  • Distribute updated copies of the Child and Family Service Care Plan (CSCP), meeting summaries, progress documentation, and task assignments to all CFT members in compliance with WISe requirements.
  • Collaborate with Youth and Family Support Specialists to identify and mobilize family support, peer support, and community resources that amplify the youth’s voice during CFT processes.
  • Complete timely WISe-required documentation, including assessments, crisis planning, and care plans, in partnership with team members.
  • Inform youth and families about their rights, including due process, and obtain necessary consents and releases of information.
  • Advocate for diversity, equity, and inclusion within CHSW and the wider community.
  • Performs other duties as assigned by the supervisor

Requirements

Responsibilities

Knowledge, Skills, And Abilities

  • Strong understanding of family support principles, early childhood and family development.
  • Proficiency in MS Office and basic office technologies.
  • Demonstrated success in community organizing, ethnic community outreach, or similar engagement work.
  • Excellent interpersonal and communication skills, with the ability to work effectively in culturally diverse environments.
  • Commitment to equity, respect, and compassionate service deliver.
  • Fluent in Spanish (preferred)

Work Environment

  • Work in locations include clients’ homes, office, and vehicle travel.
  • Exposure to typical office conditions: moderate noise, standard lighting, and occasional interruptions.
  • Home visits may include pets, residual smoke, and household hazards; smoking is prohibited during visits.
  • Frequent travel for client visits and annual professional development.

Physical Demands

  • Regularly standing, sitting, speaking, hearing, and using hands for computer and phone operations.
  • Close vision requirements for screen work.
  • Ability to sit on the floor, crouch, kneel regularly during home visits, and lift children if necessary.
  • Frequent sitting during travel.
  • Reliable and predictable attendance required.
